You may download our referral forms below to begin the process to start services. Referrals must be completed by a licensed clinician. Please send completed referrals to

In order to qualify for services, you must receive medicaid insurance and be willing to consistently meet with a therapist to further support you with meeting your goals! If you need support with getting connected with a therapist, please don’t hesitate to reach out so that we can refer you to a therapist!
